Parents/Guardians of students, planning a foreseeable absence, should complete an absence form:

Accompanying Teacher Form - Print or obtain in main office 

• Extended Absence Form is submitted electronically to South High Principal.
• Students turn in completed teacher form to office for South High Principal.

Submit form for shorter absences 1-3 days, to:

• In person to Erica Evans in the South High attendance office
• Via email to Erica Evans
• Via fax, attention Erica Evans, 612.668.4310

This form covers:

• Family obligations, activities 
• Religious observances 
• Non-Minneapolis Public School activities or events.
• College visits (two per year)
