The Minneapolis School Based Clinics is a program of the Minneapolis Health Department in collaboration with the Minneapolis School District. Please visit our website for forms and more information. Insurance companies, HMOs, and medical assistance are billed when possible. Partial payment from insurance is accepted as payment in full. CONSENT*: Students must have a clinic consent form on file at the clinic. If you do not, please stop in and pick one up. Medical - Physicals, immunizations, treatment of minor illnesses and injury, pregnancy tests, sexually transmitted disease testing and treatment, family planning exams and prescriptions. Mental Health - Prevention, early intervention, treatment and referrals. Typical issues include: depression, anxiety, stress, family and relationship issues, and planning for the future. Nutrition - Counseling for sports nutrition, special diets for weight gain/loss, vegetarian diets, and other special dietary needs. Health Education - Individual and classroom presentations on a variety of health issues.
